Title:
Application icons on side panel duplicate after restoring a previously
  hibernated session

Status in Unity:
  Fix Released
Status in “unity” package in Ubuntu:
  Fix Released

Bug description:
Application icons on (left) side panel duplicate after restoring a
  previously hibernated session.

  1) Release Ubuntu 11.10
  2) No package, this bug is with the general interface of Ubuntu
  3) Expected the previously opened application icons to show without them
duplicating
  4) The previously opened application icons are duplicated i.e. I see two Home
folder icons when there should only be one as they are grouped. If I click on
one of them it does not do anything and I am unable to remove the icon as
right-clicking on the icon and selecting Quit does not do anything. This event
occurs frequently, but not always but it would only occur if I have restored my
session after previously hibernated before.

This becomes quite irritating because it clutters up the side panel
with "dud" icons and every time I restore my session after hibernating
and this issue happens I need to rearrange the icons so that I know
which ones are the clickable ones.
