This bug report enabled me to recover quickly from a planned upgrade
(14.04 -> 16.04) that went south. FWIW I'm able to confirm that it's a
live issue.
All of our critical workstations are deployed with lvs on top of md
devices. Some, including the one I was upgrading, use md mirrors.

FWIW I'm experiencing similar symptoms on an ancient desktop that has
been stable, reliable and used every day for years. I too ran dist-
upgrade a couple of days ago. I've no idea what repos I use. That config
hasn't been touched for a long time. Can investigate if the information
might be useful.
