After restoring to a functional backup we discovered that updating from
cloud-init 22.2-0ubuntu1~18.04.3 is the cause of the problem.

   * nosuid, and noexec bits are not set on /dev
   * This has the potential for nefarious actors to use this as an avenue for

     1.Boot a Canonical Supplied EC2 instance
     2.Check the mount options for /dev.
     3.You will notice the lack of nosuid and noexec on /dev.

  [ Where problems could occur ]

   * As of 2022/10/06, I need to test this, but don't know how to build
  -aws flavored ubuntu kernels. Instructions welcome.  I'm holding off
  on adding SRU tags until I can actually get this tested.

   * If this is applied to non initramfs-less kernels it could potentially 
cause a regression for very old hardware that does nefarious things with 
memory.  For a larger discussion about that see:

   * Low risk if a driver depends on /dev allowing suid or exec this
  might prevent boot.  That being said, all kernels that have been

  I discovered that my ec2 instances based off of Canonical supplied AMI
  ami-0a23d90349664c6ee *(us-east-2), have dev mounted mounted without
  the nosuid option.

  nosuid is not found in the options list.

  I've checked the initrd, and /etc/init.d/udev script and all places I
  know of where dev gets mounted set nosuid, so it's non-obvious what
  boot code-path is being taken that results in nosuid missing.

Bug description:
  James Rice of Jump Networks noticed that there is a screen-blanker
  enabled on Ubuntu Server.

  James notes that this blanking is not enabling DPMS power saving
  (thereby negating any power-saving benefit), and is simply turning the
  screen content blank.   This means that the crash output is invisible
  which is unhelpful on a server (virtual or otherwise).

  Ideally the screen should (at a minimum) be turned on and unblanked at
  the point of an OOPs/crash being printed.

Bug description:
  [Impact]
  Multipath users using EMC XtremIO storage as boot device or at install time 
may run into this issue. With the module unavailable the device is more often 
than not unavailable. Any users changing path selector to 'queue-length' with 
other storage devices may also be affected.

  [Test case]
  1) Install on multipath system using EMC XtremIO storage / OR:
   a) Start d-i install on qemu with multipath enabled
   b) exit to d-i menu
   c) modify /etc/multipath.conf to define path selector as 'queue-length' for 
the local qemu device.
   d) restart multipathd if necessary.
  2) Try to complete the install, setting up storage as multipath and using the 
multipath device as boot disk.
  3) Reboot to disk.

  In a success case, the install should complete successfully without
  requiring manual configuration from the user to support the multipath
  storage past the normal detection of multipath and partitioning.

  In a failure case, the install may not complete, or rebooting may fail
  or lead to a system booted on a single path of the multipath device
  (ie. / on /dev/sda2 rather than /dev/mpatha2).

  [Regression Potential]
  The inclusion of a new multipath path selector driver should not cause any 
regressions, but any failure to detect, configure or boot on multipath devices 
following this change on XtremIO hardware or otherwise would constitute a 
regression potentially caused by this change.

  ---Problem Description---
  dm-queue-length module is not included in installer/initramfs

  On Ubuntu, multipath devices using the 'queue-length' path selector
  are non-functional on both the installer and initramfs environments;
  because the 'dm-queue-length' kernel module is not included in them.

  The multipath-modules.udeb (src:linux) does not include it in the installer,
  nor multipath-tools-boot (src:multipath-tools) installs it in the initramfs.

  One example is the EMC XtremIO storage, which has 'queue-length' defined as
  its path selector in the default multipath configuration, at least on 16.04.

  Other products may be affected if they are manually configured to use that
  path selector (e.g., via /etc/multipath.conf), and the mere switch of that
  might render the system _unbootable_ if booting from multipath, since the
  initramfs is affected.

  More recently this and another storage changed default path selectors out
  of 'queue-length', however, it's virtually possible for any storage system
  to be affected, with the described manual configuration change.  So, this
  change is also desired on for the next stable release, 17.04, and later.

  Patches are provided for 16.04 and 17.04.

  Just adding some information, this is not a driver specific issue, this was 
also seen with tg3 and i40e adapters . This behavior is a regression when 
compared to Ubuntu 16.04 where the interfaces where properly reconfigured upon 
bind with the information set in /etc/network/interfaces

Bug description:
  A Kernel oops leaves Ubuntu 16.04 unusable when a network bridge is
  brought down on a HPE 530SFP+ 10GBit NIC that uses bnx2x as a driver.
  This error does not appear in Ubuntu 14.04 however.

  The error is reproducible whenever issuing the commands "shutdown",
  "service networking stop" or "brctl delbr br0". Manually creating the
  bridge and subsequently bringing it down results in the same error.

Bug description:
  I am running Ubuntu 16.04 since at least four months, always with the
  nvidia proprietary drivers (installed from the repository). I used
  nvidia-361 for a while with no issues (and i need it for steam
  gaming). However, an update installed on my machine yesterday breaks
  the nvidia drivers. I tried purging the driver, rebooting and
  reinstalling and i tried a newer version for the ubuntu-drivers ppa.
  Nothing helps: With the nvidia driver installed, i get a low-
  resolution login screen, but cannot login due to missing 3d
  acceleration. Running on nouveau works fine.

  This seems very strange to since everything worked until this
  afternoon. Yesterday, i was happily playing games on steam, installed
  some updates and now, my graphics are broken.

  I am running Ubuntu 16.04 since at least four months, always with the
  nvidia proprietary drivers (installed from the repository). I used
  nvidia-361 for a while with no issues (and i need it for steam
  gaming). However, an update installed on my machine yesterday breaks
  the nvidia drivers. I tried purging the driver, rebooting and
  reinstalling and i tried a newer version for the ubuntu-drivers ppa.
  Nothing helps: With the nvidia driver installed, i get a low-
  resolution login screen, but cannot login due to missing 3d
  acceleration. Running on nouveau works fine.

  This seems very strange to since everything worked until this
  afternoon. Yesterday, i was happily playing games on steam, installed
  some updates and now, my graphics are broken.

  but cannot enter the password because no keyboard driver is present.

  This issue appears to exist in all Ubuntu releases that support EFI
  and are therefore eligible to be used as Generation 2 VMs, most
  recently tried with the April 5 ISO of the day for 16.04 (pre-
  release).

  This module *is* present when the grub menu is presented and later when the 
kernel is fully loaded.
  FYI, if some mechanism requires a mouse or touch, the hid_hyperv module will 
also be needed.

Bug description:
  At first there was no native recognition of my sound driver (see this [ 
https://bugs.launchpad.net/ubuntu/+source/alsa-driver/+bug/1525554 ] a lot of 
others a experiencing this problem)
  so there was a workaround to fix that, the notebook has sound output now, but 
ever since i got these horrible scratching noises while booting, which i dont 
have booting windows, which is kinda surprising while one could take it for a 
bios or hardware problem.

  basic workaround for fixing missing sound driver was adding two lines to the 
/etc/modprobe.d/alsa-base.conf
  options snd-hda-intel single_cmd=1
  options snd-hda-intel probe_mask=1

  with these lines a was able to get sound, but i then have these crazy
  noises while booting.

  the following are some lines from the terminal which might help:

Bug description:
  A fresh install of Server 13.10 on old hardware fails when a USB
  keyboard is present. The installation succeeds when a PS/2 keyboard is
  installed.

  The installation appears to hang at the third screen, which is Select
  Language. The screen does not respond to keyboard or mouse events.

  The issue was experienced independently on HP/Compag  dc5850 with
  Athlon X2 2.3 GHz dual core, 4GB RAM; and DELL Optiplex 755. See

  The issue was not present on Server 12.04.3 LTS or 13.04.

Bug description:
  When defaulting to the intel-pstate driver, there are only a couple of
  cpufreq governors available, none of which are handled in the
  /etc/init.d/ondemand script.

  Since this driver is meant to be used for power saving, it seems wrong
  to leave the system in the kernel default configured performance
  governor after boot, so I think it is pertinent to set this to
  powersave if it is the only suitable non-power hungry governor
  available.

  How to reproduce:

  1. enable the intel_pstate driver on a recent Intel machine
  (Sandybridge or above)

  edit /etc/default/grub, modify the following:

  GRUB_CMDLINE_LINUX_DEFAULT=quiet splash intel_pstate=enable

  and run:

  sudo update-grub

  and reboot.

  2. login

  3. wait at least 60 seconds for /etc/init.d/ondemand to complete

  check out the default cpufreq governor, you will see it is still set
  to the boot default of performance:

  cat /sys/devices/system/cpu/cpu*/cpufreq/scaling_governor

  With the attached fix the system will boot into a less aggressive
  cpufreq governor.
