Regarding #5:This icon is the symbolic icon of calendar app, as soon as we update the calendar app icon and the symbolic one this will be fixed. Regarding #3: I'm aware of bug #1362962. Here I'm talking about the small icon on the right side of the notification. This is using an old image. The latest release OTA-11 introduced an anonymous icon without a number (see screenshot on the right). Regarding #4: Notifications of missed alarms (from clock app) seems to has been introduced since OTA-11. You can see it in the screenshot in the middle. It is comming from "Wecker" which is the German wording for "Alarm". Notifications uses the icon from the app that is related for, based on the deskto file from the app. Calendar app still using the icon with number. About the alarm icon, I need to re-check but I think we do not have notifications for missed alarms. Since we do not have a proper way to handle that.
