Just thought that I would chime in here and say that this is definitely
a WiFi interference issue.  On my Dell XPS 9550 (bcm20703a1 bluetooth
chipset) I would get really bad choppy, de-synced audio sometimes.  I
finally figured out that whenever it was happening, I was connected to a
2.4 GHz WiFi network.  Connecting to a 5GHz WiFi network solved the
problem.

I have once again removed the BIOS whitelist for the WLAN cards, and
switched to a Wifi+Bluetooth Intel 7260 card.

Testing with only this cards internal bluetooth seems to work well, and
no skipping is going on. This is on a fresh Mint 19.1 install too, so
perhaps the coexistence rule works well. I will test more to see if it
reappears later on but it seems to work for now.

I will report back once I have had time to re-modify my BIOS to remove
the WiFi whitelist, and then test my Intel 7260 on there with
coexistance (I'm sure the iwlwifi implements coexistence on all wifi +
bt cards, but its there for sure on my one anyway). This may take a few
weeks, but thanks for helping me out Daniel.

Yes, Wifi/Bt coexistence is what I was thinking of. Since they're not on
the same chip in this case I can only recommend one of:

  (a) Using 5GHz wifi networks only; or

  (b) Avoid using Bluetooth audio; or

  (c) Replacing both the wifi and bluetooth cards with a single card
like an Intel 6235 which implements coexistence.

I have disabled Wi-Fi and tested a buffered video in Chrome on Youtube
(which is where I stream my music mostly from, though the stuttering and
lag was present in all other applications too), and there seems to be
almost no lagging or clicking at all. There was a lag of perhaps 100 ms
only, but nothing substantial and also not near as frequent as with Wi-
Fi on. I then tested again with Wi-Fi on, and it is stuttering again.

On my system, I use an Intel 6205 Wi-Fi card, but this card does not
have bluetooth on the same card. As seen above, my T430 has a build in
bluetooth USB device (Broadcom BCM20702). Because of this, any online
solutions about Wifi/Bt coexistance may not apply, and I have even tried
them to no effect (Adding modprobe options to enable coexistance on or
off in the iwlwifi driver)

Does the problem go away if you disable wifi?

Bug description:
  This is the same problem in
  https://bugs.launchpad.net/ubuntu/+source/pulseaudio/+bug/405294 and
  even though I have pulseaudio 12.2 (on Xubuntu 18.10), this problem
  persists (this T430 laptop has inbuilt Bluetooth).

  This makes listening to music or any audio over Bluetooth almost
  unbearable - while it is temporarily fixed by running `pactl suspend-
  sink 1 && pactl suspend-sink 0` it soon returns. I have even updated
  the BIOS but the problem persists.
