For me, this bug happened after I updated the BIOS on my Samsung Series
7 17.3 laptop. I had bought it last year and never updated the BIOS. I
went from P02 to P06 I think.

I have a new precise install on which I have the exact same problem. The
install works, as soon as I try to do an upgrade on a server that
requires a kernel upgrade I see the above problem.
GRUB_PREFIX is not set wherever it's expected to be and this causes
/etc/grub.d/05_debian_theme to fail
