Recent kernel changes are now causing this issue to appear in LXC containers
too where the libvirt mangle rule doesn't help at all.
As a result I'm now preparing SRUs for both lucid and precise so that all

1. Configure system exactly how you want it using gnome-tweak-tool and ccsm.
2. Run unity-tweak-tool and view each settings panel without changing any
settings.
What happens:
Each panel you view resets all customized settings to the unity-tweak-
tool

please see attached that dummy output is gone but sill no sound.

This is an issue that has started affecting me recently. When windows
are maximized, as some point since the reboot Unity will break and make
the window controls be unclickable if the window is maximized.
Unmaximizing the window allows me to click on them.
