I have the same problem using precision 5510 + Dell dock, used to have
perfectly working 3 monitors setup, which no longer works after the latest
update (4.4.0-77-generic).
I can no longer use 3 screen setup, 2 monitors setup is barely working and I
often need to reset it after boot, reordering
Also found no other things in Xenial and autopkgtests are fine on X
Seting v-done for xenial, but yakkety has to be checked one with the right
devices (and setup).

I haven't tried reverting http://www.spinics.net/lists/dri-devel/msg56410.html
I suspect that the root cause differs because connection between xHCI and CRTC
seems remote and the 4.9.25-gentoo kernel involved in comment #33 failed to
resume even with i915.modeset=0.

Just to add another confirmation regarding Seth Forshee's kernel, I've
be up for 11 days now with it and was crashing every 1 or 2 days (since
upgrading to Zesty).

applied. I tried also the last 4.4 on Ubuntu 16.04 (4.4.0-78-generic

It occurs with update to gdm3 3.24.1-0ubuntu0.1. Did not happen before,
downgrade to gdm3 3.24.0-0ubuntu2 solves the problem.
My session does auto-start. After the desktop is loaded I try to switch
