I had slapd 2.4.21-0ubuntu4 installed, then apt-get dist-upgrade,
which pulled in slapd 2.4.21-0ubuntu5. This modified
/etc/ldap/slapd.d/cn=config/olcDatabase={-1}frontend.ldif by adding
duplicate olcAccess lines without any {0} index
Marking won't fix for maverick as this bugs is about covering upgrades
from karmic to lucid.

While testing an SRU fix related to a release upgrade I've found out
that repositories ending with / are commented out even if
RELEASE_UPRADER_ALLOW_THIRD_PARTY is set.
The sources.lists looked like this:

I'd advise against cleaning up cn=localroot,cn=config on upgrades as
some systems may use cn=localroot,cn=config as part of their directory
ACL infrastructure. The map to cn=localroot,cn=config may also have
changed.
