To add more information to my previous report, I've recently installed
Ubuntu 18.04.1 on two machines, one of which is pretty old, and the
other is about a year and a half old.
I now have a USB storage device that causes this problem to happen every
single time I plug it into my older machine.

I managed to get this working by coldbooting the machine, then ran:
sudo update-grub # this fixes the missing GRUB config
Next I installed Vim -- APT figured out that shim-signed hadn't been
configured, and retried. This time it succeeded!
