Mine occurred after kicking off a mpd update using mpc
MPD is still running, but refusing connections. Current song finished
and then output froze.

Flash = swfplayer, but happens also with Macromedia flash
java = java6, but happens with all java versions
Extensions = adblock+, refreshblock, flashblock, disable targets for
downloads, but happens with no extensions installed.
Also happens in Suse, redhat and fedora distros.
