I think my issue is related, and I'll open a new bug if necessary, but
pulse thinks that my headphones are forever plugged in. It randomly
goes back to normal. Also, Sound only displays either Speakers
Built-in Audio or Headphones Built-in Audio, never both when both are
plugged in.
