When PXE booting grub2 with a proxy DHCP server, the next-server is
never read from the proxy dhcp server. It is only read from the normal
dhcp server, therefore net_default_server does not get set and makes
grub2 unusable with a proxy dhcp server.

Ok, so I've tracked this down to mpd picking up a different version of
libavutil which I built some time ago from source:

it's picked this up in preference to the stock version in
/usr/lib/x86_64-linux-gnu/
Well I've only just completed the update to 14.04 which took a few days
to get sorted so I'd like to have that running and stable for a while
first before contemplating the next one. Only additional thing I can add
at this point is that it was working fine in 12.04.

This problem looks almost identical to 519787 however that is somewhat
old now and is marked as fixed - of course that's not to say something
of a similar ilk has crept back in again.
The problem is repeatable, on an attempt to play any m4a audio files,
mpd segfaults inside
I've performed the upgrade from 12.04 to 14.04 over the Christmas break
which has restored all the repositories back to default (I was using the
PPA from Rolf's PPA to work round this) and can confirm it's all working
now. There's still the massive delay on the start of the initial scan
(as
I updated the bios to A07 as this is the latest listed for my service tag
1VMBS61.
I updated the BMC bios and the SAS controller BIOS.
Still won't boot with kernel 4.4.0-57.

I tried some other kernel parameters:
scsi_scan=sync -> didn't help
pci=nomsi,nommconf -> didn't help
For now I'm just booting the 3.13.0 kernel since I need the server up
and don't have good physical access to it.
