Same issue here on Dell E6530 with kernel 3.11.0-13-generic, but it also
occured with previous versions of the Kernel / Ubuntu. Messages from
dmesg attached.

I've seen comment #12 but I still reported the issue here as it seems to
be the same hardware involved (USB controller Intel Corporation 7
Series/C210 Series Chipset Family USB xHCI Host Controller):
