Still seeing this on 16.04 LTS as well -- the sound settings menu does
not clean up HDMI sound sources that have been removed.

sadly there still seems no solution for this bug.
But I found a workaround. start
pavucontrol
via terminal and switch in the first tab the output device to Internal Audio.
Worked for me.
