The big difference that I am seeing between the 2 versions is that the KDE 3 version is using .menu files whereas the KDE 4 version is using .desktop files. I am not 100% on how Gnome reacts with the .menu files, but still, we need to look into this as KDE 3 isn't going anywhere any time soon.

I can confirm this issue, however the likelihood of it getting fixed is slim-to-non with the KDE 3 System Settings as it would require a substantial amount of code rewrite at this time. If there is a slim chance on getting this fixed, I will look further into it after further discussions on IRC.
