Public bug reported:

After an fresh install of ubuntu 14.04.1 on an luks encrypted root - everything
worked fine.
after the first reeboot i made an "apt-get dist-upgrade" and after the ext
reboot the system won accept my luks passphrase.

After further investigation we found an similar bug in ubuntu from 2013
- where in initrd was created with a missing "cryptsetup" - so i tried
the old kernel and it worked...

so it seems that in this kernel the "cryptsetup" is missing...

what to do?
