Can you do a 'cryptsetup luksDump' on the device?
Its just not possible that a device gets uncrypted without any password or
keyfile.

Trying to fulfill your request led me to figure out what is happening,
and why it is my dumb mistake, no creepy, crawly bug here.
What I meant about no passphrase actually is, you need to fill it the
first time, then after that the passphrase is not asked for. This
latter was what freaked me
