Affects me after upgrading from 15.10 to 16.04. Also one of my two
external screens goes black and wont come back until I reboot. Please
let me know what else you need in order to find the issue.

I experience the same problem. Can reproduce it the same way as re1nald0 wrote
on 2015-05-24:
Open applications → screen lock → wait some minutes → unlock screen appears two
times → all applications are closed

I have this same problem. It has happened quite rarely before but
nowadays more often and today it has happened twice. It's quite a show
stopper when you have to start all the applications three times in a
same workday cause there's bunch of them.
With my laptop this won't happen if you just lock
[Expired for Ubuntu because there has been no activity for 60 days.]

Yes, of course, it is reproducible. Here are the steps:
1. Open some applications (Nautilus, Chrome, etc.)
2. Click the system menu in the menu bar and select Lock.
3. Unlock the screen. Those applications disappeared and sometimes we need to
enter password twice.
