Of course i'm not sure whether this is a PA problem or an ALSA or UDEV
bug.

Ubuntu 14.04.2 LTS
Pulseaudio 1:4.0-0ubuntu11.1

*  works almost as expected: there is one physical input and one output,
but in ALSA one input and two outputs appear.
