Public bug reported:

no opportunity to reach the console or rescue the install.  Something
gets installed and the operating system is there.  I'm going to try a
repair console to the boot record installed.  This has happened 10+
times, I kept thinking it was a hardware problem.  Now all hardware is
checked out operational.
