Looks like this was an fglrx problem. I installed the ATI driver from
source, following the instructions on:
http://wiki.cchtml.com/index.php/Ubuntu_Gutsy_Installation_Guide#Method_2:_Install_the_Catalyst_7.11_Driver_Manually
After that lock screen works correctly.

With the new ATI Catalyst 7.11 driver installed:
* Ctrl-Alt-L locks the screen correctly.
* The screen saver works, starting up after the time-out period and locking
the screen when lock screen is checked.
* 3D effects work.
However:
