I just got hit by this issue too. It looks like the workaround is to
uncheck the "blink task bar" notifications in hexchat.
It seems gnome-shell isn't handling the "blink task bar" notifications
properly.

If the Hexchat window is not in focus, Hexchat will display a
notification with the name of the Hexchat window in quotes and 'is
ready' (eg, '"Avital_ @ freenode / ##linux - Hexchat" is ready') for
- every message received in the channel that is currently selected.
I believe this would be a GNOME-Shell bug and I vaguely remember it
being fixed.
