I have come up with the attached patch to fix e2fsck to handle the old
inodes left behind when you use lazy_itable_init. It simply checks if
ctime s_mkfs_time and offers to clear the inode. The check is skipped
if the clock is detected to be broken. The first prompt informs you

I have the same problem. Nvidia raid0, didn't have problems with it before,
just now.
The error doesn't happen during boot, but only when I started a 'dd' test,
reading through the whole array:

Do they happen every boot, or only sometimes?
You appear to have two different raid sets, is this correct? Can you post the
output of sudo dmraid -s?

then you have left over fake raid signatures on the drives that you
should remove with dmraid -E.
