Shouldn't the exchange server type not be shown if exchange operations
plugin is not enabled?
Also, enabling the exchange operations plugin did not help at all for
me.

Looks like enabling Exchange Operation plugin inside Evolution solves
the problem.
I don't know why but this plugin is disabled by default. You may enable
it by selecting Edit/Plugins menu entry from Evolution.

I get the same problem with fiesty. When i try to create ou edit an
exchange account, i can see only the username.
Package in use:
evolution - 2.9.6-0ubuntu1
evolution-common - 2.9.6-0ubuntu1
evolution-data-server - 1.9.6.1-0ubuntu2
evolution-data-server-common - 1.9.6.1-0ubuntu2
how about 2.9 fiesty version?

* debian/patches/10_pkg-config-rpath.patch:
- patch from Debian, Add explicit -rpath to
private libs pkg-config to fix .la-less builds against these libs,
fix evolution-exchange not running
Run:
export LD_LIBRARY_PATH=/usr/lib/evolution/2.8/
/usr/lib/evolution/2.8/evolution-exchange-storage
in a terminal as a workaround to start the exchange backend. Clearly
this is some sort of rpath issue in the build

Though the calendar backend still doesn't work using that workaround,
but you can at least get mail.
