Public bug reported:
I originally susbscribed to bug #60621 as I have the same symptoms
exactly (lack of splash and same error message from usplash), but the
bug title has apparrently been renamed and now only relates to halting
the system, and wide screens, none of which relates to my case, since
the lack of splash is during startup, and I don't have a wide screen,
but just an old CRT monitor.
The error message is the same then, reproduced below:

System is starting up...
usplash: no usable theme found for 640x480
screen init failed
-------------
... then the boot sequence continues as normal and everything works
fine.
