I can confirm the bug on 64-bit 12.10. In addition of the pulseaudio not
working, the networking is also down until I unplug the webcam, so there
might be bug at the kernel level also.

I am also seeing this behavior with a logitech HD Webcam C310. I am
running ubuntu 12.10 and am receiving the same initialization failed
error. As soon as I unplugged my webcam I regained the ability to use my
speakers, and that ability remained even after plugging the webcam back
in.

I've had the same issue with Ubuntu 11.10 and also in 12.04 with
pulseaudio version 1.1-0ubuntu15, both 64-bit. It occurs about 90% of
every suspend/resume when usb webcam (Logitech C270) is plugged in.
After reboot everything works normally.
