1. This bug happened during a Linux Mint installation, not Ubuntu
2. Looking at your syslog, it appears that you got plenty of I/O errors from
reading your installation media.
You either have a bad media or a faulty drive.
You can try again with another CD/DVD and burn it at a slower speed.
