I'm using TwinView on Oneiric final, and it seems to happen to me all
the time on the primary monitor. The indicators work sometimes, but most
of the time they don't.
The secondary monitor works fine.

I'm having the same issue, I made a video to illustrate the problem.
However gtk recordmydesktop was giving me a hard time recording both of
my screens so there are some corruptions, most noticeable the fact that
only the left upper side of the menus show. However it does show the
problem.

It seems to work after the latest batch of updates from oneiric-proposed
(containing unity 4.24). Can someone else confirm this?

I have also hit this in unity-2d, although it seems to be harder to
reproduce it in unity-2d. If you click around the indicators on the
main screen a few times in unity-2d it starts working normally again.
On the secondary screen the menus display reliably just like they do on
unity 3d for me.

Confirming this after the latest updates. A reboot didn't fix it but it
seems to have gone during the same session it has previously occurred in
so it's not a constant issue for me.
