Alt-tab focuses wrong window with "Keep on top"

I think the "Keep on top" step is unnecessary for this to happen. My
daily workflow involves:
- irssi in a terminal window on a second monitor, snapped to the left.
- ssh into a virtual machine on my main laptop screen, maximised
- Firefox on my main laptop screen, maximised
