I'm afraid I don't have a procedure to reproduce the issue, but I can
confirm that I still see it regularly.

I'm not sure about it either, but I THINK it is because of a
multimonitor setup. Which GPU are you using, which driver, and can you
also post your /var/log/Xorg.0.log file, /var/log/syslog as well as
/home/yourusername/.xsession-errors ?
