added fdisk -l of the sd card

when i recreate a new partition table during the partitioning step in the
installer, it works and fdisk -l looks like this:
Disk /dev/mmcblk0: 4035 MB, 4035969024 bytes
4 heads, 16 sectors/track, 123168 cylinders, total 7882752 sectors
Units = sectors of 1 * 512 = 512 bytes

I think we've worked this out on IRC as more of a
documentation/confusion issue with regard to what bits are what in the
d-i image directories.
