The culprit is revision 4187 
(http://bazaar.launchpad.net/~unity-team/unity/trunk/revision/4187): in 
lockscreen::Controller a new menu::Manager instance is created to handle the 
lockscreen indicators, then as soon the new LockScreenDBusIndicators instance 
connects to the new com.canonical.Unity.Panel.Service.LockScreen bus it calls 
DBusIndicators::Impl::OnIconsPathChanged, which asks the lockscreen panel 
service for icon paths, but the *lockscreen* panel service is just started and 
doesn't know any of the new paths needed for the indicators which provide 
custom icons, so it just reports the default icon theme search paths. An 
icon_paths_changed signal is emitted which invokes 
menu::Manager::Impl::IconPathsChanged that sets the search paths for the 
default GTK icon theme (which is a global shared resource of the process).
After unlocking the screen no signal is sent to recover the proper search 
paths, so some indicators shows the missing icon.
Maybe a better way to handle this whole indicators icon thing would be to have 
a different GTK icon theme object for each menu::Manager instance instead of 
modifying the global default icon theme?

This is affecting me I believe, but with new distros. In 14.04 LTS I had
audio (and I can't remember if I had to play around with stuff to get it
working), but when I upgraded to 16.04 LTS it no longer showed up.

I have a Nvidia GTX 970 and the hdmi port connected to my TV has sound,
but the display port connected to my Dell U3416W monitor does not work.

The system for this bug is running Trusty with xvfb 2:1.15.1-0ubuntu2.7

Xvfb fails to launch after being used for a couple days and I can't see
any reason to why it wont launch.

We have inetd.conf setup to run an x11vnc script with the following
command whenever a connection comes in on port 5902 (assumed to be a
tunneled VNC).

I actually discovered the root cause.  GnuTLS doesn't work with iOS.

I should note a couple things:

For an existing file with rather complex EPS files from Mathematica:
black rectangle, bounding box is read just fine.

For a circle drawn in xfig, imported in EPS or PDF:  empty rectangle,
bounding box doesn't work (present in EPS header)

Importing a second copy of an EPS file that's already present and works
(first case):  bounding box fails on the newly added EPS only.
