Well, the system was in an inconsistent state after the attempted
upgrade from 14.04 to 16.04.
I ran some combination of
sudo apt-get update –fix-missing
sudo dpkg –configure -a
sudo apt-get install -f
which prompted me with "Yes, do as I say" and appears to have removed
many packages.

Booting with the kernel option nvme_core.default_ps_max_latency_us=11000
makes a difference for me. i no longer see filesystem crashes with this
option on 4.10.x and 4.8.0-49 kernel. Prior to this kernel change,
filesytem crashed soon after finishing the boot process.

I am running an Ubuntu 16.04 server running on an HP Proliant chassis
with several lxc containers running on zfs. While performing a series
of debian updates on the host and container, as well as container
destruction/creation, the kernel spits out a stack trace indicating

I was asked why I usually try to test too much, that is the reason :-/
Rushing it once and failing instantly.
But hey our regular jenkins based testing found it - at least.
Still that means probably overall 1-2 days of broken guests :-/.

We observed severe data loss/filesystem corruption when executing fstrim
on a filesystem hosted on an Eternus DX600 S3 system.
There is multipathing via a fibre channel fabrics but the issue could be
reproduced when disabling multipathing and using one of the block
devices
