Note that the fix is a random crash fix. I didn't get the crash myself
but upstream applied the fix upstream, which is supposed to remove the
crash.

As testing it, I didn't get any new crash at least, marking as
verification-done.

Thanks Josh for the extended checks, I have tagged it up for Bionic
correctly and sponsored it for the review of the SRU [1] Team. Once they
accept it into Bionic-proposed they will ask for a verification, it
would be great if you again could check then that it works. It could,

   * c) I happen to know that for DPDK usage of these devices several fixes
     in later rdma-core are preferred or even needed. I'm not sure about the
     usage in this case - but enabling could expose those issues which
     formerly were hidden behind the "not supported" misdetection of the
     card.
  
  For B) and C) I'd want Microsoft to ack and test this from a PPA and do
  the verification on this - to be not only ok for this but for for Azure
  in general.

It looks like items are in rare cases being removed from the export
cache's hash table without being removed from the queue. Once they are
removed from the hash table, no downcall can complete the request,
because the item can't be looked up in the hash table, leaving the item
orphaned, along with its reference to the filesystem we are trying to
unmount. It looks like this has already been fixed upstream in
https://github.com/torvalds/linux/commit/4ecd55ea074217473f94cfee21bb72864d39f8d7,
and that commit has been pulled into an upcoming Ubuntu kernel release.

I did confirm it on bionic as a kernel issue - I could recreate the error on 
bionic with the bionic proposed qemu and the disco kernel (and additionally 
with an older bionic kernel, too).  I wasn't able to get a setup for disco to

It looks like modesetting, or at least a destructive probe of the
attached displays.

There's nothing KMS related in dmesg corresponding to the blanking, so I
don't think it's an invalid FB (I'd expect i915 to spit something in
dmesg in that case)

Thanks. That doesn't seem to show any single recurring problem though.
So it looks like log file flushing is not the explanation for this bug.

I wonder if disk performance is a factor. Can you please run:

  lsblk

and

  sudo hdparm -I /dev/sda

and send the output?

We used BPF/BCC to trace all refcount changes on all of the relevant
mounts, and for each (struct mount*, stack) pair, kept track of the
number of references acquired/released by that pair. Including the
struct mount* in the key in our map make the map quite large, but
allowed us to pare the output down to a manageable size when we hit this
issue by filtering out all of the refcount changes except those related
to the one affected filesystem.

Looking at all the reference count changes for a particular filesystem
affected by this issue, we can see that the extra reference which is
acquired but never released is being taken by code related to the NFSD
export cache. In particular, the cache maintains a queue of requests
which rpc.mountd is supposed to read and process. After processing a
request, rpc.mountd is supposed to issue a downcall with the info
requested by the kernel, at which point the item can be removed from the
queue, and can be flushed when it expires, releasing the reference the
item holds on the mount.

rpc.mountd is waiting for requests, as it should be, but the kernel is
not notifying mountd that there are requests in the queue. We have more
investigation to do to understand how the kernel got into this state.

On this Kaby Lake G laptop, GNOME Shell blanks the screen for a second
each time the AMD GPU is powered up. Triggers for powering up include:
launching something which loads GL such as Steam or glxinfo, loading the
Display configuration page, and so on.

This behaviour does *not* occur with GNOME Shell under X, so it's
probably something to do with mutter's KMS interactions.

Because the GPU powers down quite aggressively (I think it's about 5sec
without activity) this can be quite annoying :)

Fixed via ubuntu-mate-meta 1.249.

xinit was pulling in gnome-terminal, Adding xterm to the Ubuntu MATE
seeds and rebuilding ubuntu-mate-meta has resolved the issue.

I don't actually understand why though, because xinit has `Recommends:
xserver-xorg | xserver, gnome-terminal | xterm | x-session-manager | x
-window-manager | x-terminal-emulator`

  * mate-session-manager provides x-session-manager
  * marco (MATE Window manager) provides x-window-manager
  * mate-terminal provides x-terminal-emulator

So why did seeding xterm fix this?

I ran into this issue today as well.  Performing the following can
recreate the issue on Ubuntu 18.04.3 LTS:

When running 'apport-cli --hanging $(pidof gnome-shell)' in a ssh shell,
it wants to spawn a browser to login to launchpad.net; since it did not,
I saved the apport and linked it here. After I ran the killall command
and ran the ubuntu-bug command it did not show an ID to link here.

I have a problem that looks very similar to this one, but it's happening
with linux-firmware 1.173.9 and only on my home 5 GHz SSID (the 2.4 GHz
SSID connects fine). What do I need to send you in order to have it
fixed?

I started getting this after getting a new router. The router I had
before didn't trigger this.

ubuntustudio-performance-tweaks wasn't installed until Erich suggested
it earlier, so I don't think it was causing the problem.

I don't have jack or cadence auto starting.

I'm not sure where else to look. I think the hardest thing in diagnosing
this is that sometimes it happens and sometimes it doesn't.

I have another computer which I'll eventually reisntall everything and
hopefully I'll know more

Ok, the bileto systemd package seems to look sane, although it's a
really hard thing to review. The tests not being green seems reassuring
I'd say. I somewhat agree that having both 242 and 243 rc in already
-proposed at one point does seem like a plus, but on the other side
their removal means there were unsuspected regressions. That being said,
I am willing to take the risk so early after Feature Freeze.

Let's make sure to check if the regressions we saw with 243 and 242
aren't visible here (if those weren't visible via autopkgtests of
course). Anyway, +1 on the FFe. Please upload as soon as possible.

I've done some upgrades, since I posted the original report, but after
uninstalling and reinstalling K3B and Wodim, the symptoms are the same
(partial log output, with error, below).  The "chmod +r /usr/bin/wodim"
trick still functions effectively as a workaround.  Please let me know
if you need anything else.

With this ticket opened against disco (still in service, compared to LP 1841488 
which is about cosmic, that's no longer in service) I suggest that we close LP 
1841488 (set to Invalid) and work on these patches based on this ticket (LP 
1841984).

Nevertheless, the  disco master-next tree already includes all patches
(they probably came is via a stable upstream release update):
