[Bug 1826447] [NEW] Azure: Backport vIOMMU driver (increase vCPU limits)

Description:

To support VM instances with >255 vCPUS, we need the patch set below
backported to the Azure 4.18 kernel. These patches include -

  - New Hyper-V IOMMU driver. This driver will be Hyper-V specific.
  - Minor update to the Hyper-V specific virtual PCI code. Need to expand the 
size of an array to allow for a larger number of vCPUs.

There are very large VM instance sizes coming to Azure in 2019, and
Ubuntu will require the following patches in order to function properly:

Some SSD drives made by LiteOn can crash the kernel due to bad drivers.

At a high level the root cause of this problem is that the device only supports 
commands with a transfer size up to 512KB.  This worked fine with older 
versions of Linux but newer versions are attempting commands with a transfer 
size of 1280KB which is problematic for this device (LITEON EP1-KB480)

1) Create a VM in Azure (any size)
2) Update cloud-init to the latest in xenial-proposed
3) Capture VM image
4) Provision a new VM using the following cloud-config:

As an additional test case I've tried to use the steps as described here
to re-partition the ephemeral disk:

Using your branch from this bug the sult is that it appears to fail to
repartition but it does format/mount the disks. Here are my repro steps

 $ azure group create ubtest 'east us'

 $ azure vm create ubtest ubtest-1604 Linux --image-urn
canonical:UbuntuServer:16.04-LTS:latest --admin-username azureuser
--admin-password '' --nic-name ubtest-1604 --location "East
US" --vnet-name "ubtest" --vnet-address-prefix "10.0.0.0/24" --vnet-
subnet-name "ubtest" --vnet-subnet-address-prefix "10.0.0.0/24"
--public-ip-name "ubtest" --public-ip-domain-name "ubtest" --custom-data
./customdata.txt -z 'Standard_L32s'

The customdata.txt file contains something like the following:

 disk_setup:
 ephemeral0:
 table_type: gpt
 layout: [66, [33, 82]]
 overwrite: True

Result with default 16.04 image: /mnt is not mounted and we see a failure in 
the cloud-init.log (attached).

Results with branch from this bug: /mnt and swap space are created, but
the partition layout is not applied. So part1 is still type "Microsoft
reserved" and only 120MB, and part2 is type "Microsoft basic data" and
is a 5.5TB swap partition :)

Maybe there's something wrong with my cloud-config?  Do I need to use a
different layout format when table_type is 'gpt'?

Hi Scott,

The linked branch seems to reliably format the ephemeral disk on large
instances. However, after redeploying the VM I don't see the ephemeral
disk mounted after boot. The cloud-init log does not indicate any error,
and if I type 'mount -a' (or do a simple reboot) then the disk is
mounted properly with an ext4 filesystem.

To test this I'm creating an L32s VM on Azure with Ubuntu 16.04,
updating cloud-init manually from your git branch, and then I click the
'redeploy' option via the portal.  I see that the ephemeral disk is
partitioned/formatted correctly, but just not mounted.  Am I missing
some configuration?  What is normally responsible for mounting the
ephemeral disk once it is formatted?

GPT formatted ephemeral disks have two partitions (unfortunately). The
first is the Microsoft reserved partition
(https://en.wikipedia.org/wiki/Microsoft_Reserved_Partition). This
appears to be tripping up can_dev_be_reformatted().

Function address_ephemeral_resize() will first check if
is_new_instance=True, and if so it will happily format the ephemeral
disk.  That's why this works at first boot.

For subsequent boots where is_new_instance=False,
address_ephemeral_resize() will call can_dev_be_reformatted(). This
function sees that the ephemeral disk has two partitions and returns
false:

Aside from the TypeError, even if we comment this part there is a later check 
to ensure that this partition is NTFS formatted (which it isn't). This will 
also return false and you will see the following error in the logs:

 "reformattable=False: partition 1 (/dev/disk/cloud/azure_resource-part1
-> /dev/sdb1) on device /dev/disk/cloud/azure_resource was not ntfs
formatted"

I think the correct behavior should be for cloud-init to detect that
this is a GPT formatted ephemeral disk, or check that azure_resource-
part1 is a "Microsoft reserved partition".  And if so skip
azure_resource-part1 and focus on azure_resource-part2 instead to
determine if it is safe to reformat.

Some Azure instances such as L32 or G5 have very large ephemeral disks
which are partitioned via GPT vs. smaller ephemeral disks that have dos
disklabels.

At first boot of an instance the ephemeral disk is prepared and
formatted properly. But if the instance is deallocated and then
reallocated (thus receiving a new ephemeral disk) then cloud-init does
not handle reformatting GPT partition ephemeral disks properly.
Therefore /mnt is never mounted again.

Test cases:
 1. Deploy an L32(s) VM on Azure
 2. Log in and ensure that the ephemeral disk is formatted and mounted to /mnt
 3. Via the portal you can "Redeploy" the VM to a new Azure Host (or 
alternatively stop and deallocate the VM for some time, and then 
restart/reallocate the VM).

Expected Results:

  Some Azure instances such as L32 or G5 have very large ephemeral disks
  which are partitioned via GPT vs. smaller ephemeral disks that have dos
  disklabels.
  
  At first boot of an instance the ephemeral disk is prepared and
  formatted properly. But if the instance is deallocated and then
  reallocated (thus receiving a new ephemeral disk) then cloud-init does
  not handle reformatting GPT partition ephemeral disks properly.
  Therefore /mnt is never mounted again.

The test looks good, here are my repro steps:

 1) Create an Ubuntu 16.04 VM in Azure
 2) Attach 128GB data disk
 3) Partition and format the disk
# wipefs --all /dev/sdc
# sgdisk -Z /dev/sdc
# sgdisk -n '1:2048::264191' /dev/sdc
# mkfs.ext4 /dev/sdc1

Expected result: mkfs should return very quickly because
lazy_itable_init is enabled.

Actual test results:

  - Kernel 4.4.0-71.92: mkfs blocks and formatting takes about
8-minutes.

  - Kernel 4.4.0-73.94~lp1682215: mkfs returns immediately.

block: relax check on sg gap
If the last bvec of the 1st bio and the 1st bvec of the next
bio are physically contigious, and the latter can be merged
to last segment of the 1st bio, we should think they don't
violate sg gap(or virt boundary) limit.

Both Vitaly and Dexuan reported lots of unmergeable small bios
are observed when running mkfs on Hyper-V virtual storage, and
performance becomes quite low. This patch fixes that performance
issue.

The same issue should exist on NVMe, since it sets virt boundary too.

time.  Commit 4fcd1813e640 mostly resolved this issue and is already
integrated into the Xenial kernel.

[Bug 1376274] Re: unscd segfaults regularly

I've tested unscd with the patch and saw no issues, but unfortunately
there is no simple repro script that generates the failure. Customers
just see this issue appear after some time of running the service.

The customer in comment #3 also confirmed that unscd_0.52-1 fixes the
issue for them, and this null pointer check is the only change in that
release. To me this seems low risk to keep the null pointer check and
release this.

Hi Scott,

Thanks for testing. I tested again (~10 times) with the latest Trusty
image and I wasn't able to reproduce the issue either. I'm not sure what
happened, if it's some timing issue or was just typo on my part :/  I
did not keep the repro VM around, unfortunately.

In that case I'm OK calling this resolved since I'm unable to repro
again.

I'm following the instructions to send cloud-config and repartition the
ephemeral disk in Azure: https://wiki.ubuntu.com/AzureSwapPartitions.
These instructions fail on Ubuntu 14.04 and 16.04 (did not test 12.04
yet).

Repro:

1) Add the following content to a file such as customdata.txt:

 #cloud-config
 disk_setup:
 ephemeral0:
 table_type: mbr
 layout: [66, [33, 82]]
 overwrite: True
 fs_setup:
 - device: ephemeral0.1
   filesystem: ext4
 - device: ephemeral0.2
   filesystem: swap
 mounts:
 - ["ephemeral0.1", "/mnt2"]
 - ["ephemeral0.2", "none", "swap", "sw", "0", "0"]

2) Create a VM in Azure with this cloud-config:

 $ azure group create ubtest 'east us'

 $ azure vm create ubtest ubtest-1604 Linux --image-urn
canonical:UbuntuServer:16.04.0-LTS:latest --admin-username azureuser
--admin-password '' --nic-name ubtest-1604 --location "East

Expected Result:

 - /dev/disk/cloud/azure_resource should have two partitions; one ext4 and a 
second swap
 - /dev/disk/cloud/azure_resource-part1 should be mounted on /mnt2
 - /dev/disk/cloud/azure_resource-part2 should be active swap space
 - /etc/fstab should be configured correctly


Actual Results:

 - /dev/disk/cloud/azure_resource-part1 is properly mounted on /mnt2.
 - On Ubuntu 14.04, /dev/disk/cloud/azure_resource is re-partitioned as 
expected.
 - On Ubuntu 16.04, /dev/disk/cloud/azure_resource is *not* re-partitioned. 
There is only a single partition (no swap space).
 - On Ubuntu 14.04 and 16.04, /etc/fstab includes an incorrect entry for the 
swap space:

   ephemeral0.2noneswapsw,comment=cloudconfig  0   0

This clearly won't work, it should resolve to "/dev/disk/cloud
/azure_resource-part2".  I do notice that on Ubuntu 14.04, the ephemeral
disk is re-partitioned correctly, but there is no symlink in
/dev/disk/azure/ to the second partition. These symlinks are created by
/lib/udev/rules.d/66-azure-ephemeral.rules. Perhaps cloud-init needs to
re-trigger this udev rule after repartitioning the disk.


FYI, I also tested with the latest cloud-init that is currently in 
xenial-proposed (0.7.8-47-gb6561a1-0ubuntu1~16.04.1) with the same results.

Hi Scott,

I think we would actually like to move to a world where cloud-init can
function independently of the agent and does not start it.  The
walinuxagent service can then start after cloud-init is finished, and
function as a guest agent to handle things like VM extensions, etc.

This behavior of cloud-init starting walinuxagent was needed early on to
allow the agent to communicate with the wire server so that the
provisioning ISO could be attached to the VM.  That's no longer needed.
All cloud-init needs to do is obtain the certificates for the SSH public
key and post 'ready' to the fabric once it has processed its cloud-
config.

When using user management tools such as usermod we see spurious
messages due to unscd, for example:

  # usermod -a -G testgroup username
  sent invalidate(passwd) request, exiting
  sent invalidate(group) request, exiting
  sent invalidate(passwd) request, exiting
  sent invalidate(group) request, exiting

The command completes successfully, but the spurious message looks like
an error and causes concern among our users. This issue was apparently
fixed in Debian here: https://bugs.debian.org/cgi-
bin/bugreport.cgi?bug=622230. Can Ubuntu please take this fix?

During provisioning cloud-init adds an entry for the ephemeral disk in
/etc/fstab. After provisioning this entry is correct and points to
"/dev/disk/azure/resource-part1". This symlink is created dynamically by
66-azure-storage.rules.

For some reason after the first reboot cloud-init overwrites the fstab
entry and changes the "/dev/disk/azure/resource-part1" to the device
name that it points to, i.e. /dev/sdb1.  However, this is incorrect
since /dev/sd* device names are not persistent.


Repro:

1) Provision an Ubuntu VM on Azure (I tested with 14.04.4)
2) The fstab entry for the ephemeral disk (/mnt) correctly points to 
"/dev/disk/azure/resource-part1".
3) Reboot the VM (sudo reboot)
4) The fstab entry now incorrectly points to /dev/sdb1 instead of the symlink.

Impact:
There is a chance that the customer's ephemeral disk will not be mounted 
properly if the device names change after a reboot.

The latest walinuxagent 2.0.16-0ubuntu2~12.04.0 package for Precise
included the postinst file from the Trusty package. The postinst script
configures dhclient.conf so that the DHCP client sends the system's
current hostname to the DHCP server. Unfortunately, the dhclient.conf
configuration is different for Precise and Trusty, and thus this
functionality regressed in Precise.

The solution is to just replace the postinst file in the Precise package
for the 2.0.16 walinuxagent with one from an older/correct package, and
rebuild the package.  If it helps I've attached the correct postinst
file for Precise to this bug.


Repro:
 - Create an Ubuntu 12.04 VM in Azure
 - Upgrade to the latest walinuxagent package for Precise 
(2.0.16-0ubuntu2~12.04.0)
 - Change the hostname
 - Run "sudo ifdown eth0 && sudo ifup eth0"
 - After a few seconds run "hostname -f"

Expected: Hostname is resolvable.

Result: Hostname is not resolvable because the new hostname was not sent
to the DHCP server, because the dhclient.conf configuration is
incorrect.

Provisioning with a user password is supported on Azure, however it
seems the behavior of cloud-init is to not set the user password if the
user already exists.  Ideally, if the user exists, then during
provisioning cloud-init will read the user password from the ovf-env.xml
file as usual and attempt to set that password for the existing user.

Repro:
1) Capture or upload a user image with a pre-created user
2) Provision a VM with a password

Expected: password is (re)set on the user account based on the info in the 
ovf-env.xml file.
Result: password is not set.

On Azure, cloud-init relies on the system-uuid as based by SMBIOS a
unique ID for a cloud instance.  If this ID ever changes, then cloud-
init will attempt to reprovision the VM.

This recent kernel patch in the Ubuntu kernel corrects the endianness
for some SMBIOS fields, but also has the effect causing cloud-init to
think that the system-uuid has changed:

The impact is that cloud-init attempts to reprovision the VM, often

On the latest Trusty package it appears that the upstart file
"ephemeral-disk-warning.conf" still contains and older version of this
script that detects the ephemeral disk by grepping for "sdb1".  It
should instead run ephemeral-disk-warning.sh like what the systemd
service does.

Also, I'm not sure there is a strong need to "chattr +i" this file since
it will be recreated at boot time if needed (the Linux agent doesn't
chattr this file).  What's happening now is customers are finding this
"DATALOSS_WARNING_README.txt" file on partitions other than /mnt
(because "sdb1" as the resource disk is not persistent) and then don't
understand why they cannot delete it :)

I tested cloud-init 0.7.5-0ubuntu1.15 from -proposed for Trusty on Azure
and confirmed that the new package resolves the issue.

The upgrade path is also working as expected.  I first created an Ubuntu
14.04 VM using ARM.  After shutting down the VM via the Azure portal
(portal.azure.com) and then restarting it I saw that the Instance ID
changed (you can see this in /var/log/waagent.log).  However, the
symlink in /var/lib/cloud/instances/ that links the old Instance ID to
the VM unique ID had been created, and so cloud-init did not attempt to
reprovision the VM.

and confirmed that the new package resolves the issue.

The upgrade path is also working as expected.  I first created an Ubuntu
14.04 VM using ARM.  After shutting down the VM via the Azure portal
(portal.azure.com) and then restarting it I saw that the Instance ID
changed (you can see this in /var/log/waagent.log).  However, the
symlink in /var/lib/cloud/instances/ that links the old Instance ID to
the VM unique ID had been created, and so cloud-init did not attempt to
reprovision the VM.

Large sized VMs with 32 VCPUs and more than 100G memory sometimes hang at boot,

In Azure, the DHCP client on Ubuntu VMs is configured to push the
current hostname to the DHCP server, which then publishes the hostname
into Azure DNS.  However, after changing the hostname and rebooting, the
old hostname is always pushed to Azure DNS instead of the new one.

Repro steps:

  - Create Ubuntu 14.04 VM on Azure
  - Edit /etc/hostname to change the hostname
  - Reboot the VM
  - After reboot, the hostname will be changed, but commands like 'hostname -f' 
and 'sudo' will complain that the new hostname cannot be found via a DNS lookup.

Next, bounce DHCP to force the DNS update and check that the new
hostname has been published:

  - Run "sudo ifdown eth0 && sudo ifup eth0"
  - After a few seconds run: "host NEWHOSTNAME".

DNS lookup to the new hostname should succeed now indicating that the
new hostname was pushed to the DHCP server and into Azure DNS.  Commands
like 'hostname -f' and 'sudo' will work now.

init/+bug/1292648).  So this appears to be a regression.

We'll test this new kernel.  It appears the directory
/~jsalisbury/lp1445195/utopic is empty, but it looks like the kernel
was placed in the /~jsalisbury/lp1445195 directory instead.  I assume

expected.  So let's please ack these patches for the next Vivid kernel.

This issue impacts Ubuntu 12.04.

Whenever a device is unregistered in vmbus_device_unregister
(drivers/hv/vmbus_drv.c), the device name in the log message may contain
garbage as the memory has already been freed by the time pr_info is
called.  Log example:

  [ 3149.170475] hv_vmbus: child device àõsèè0_5 unregistered

By logging the message just before calling device_unregister, the
correct device name is printed:

  [ 3145.034652] hv_vmbus: child device vmbus_0_5 unregistered


This issue has been resolved with the upstream patch:

In case customers are not familiar with the term Custom Data or how to use 
it, I would recommend linking to our documentation:

While passing a cloud-config via CustomData will work great during the
initial setup and provisioning of the machine, is there a workflow that
could allow a customer to configure a local cloud-config to configure
swap space post-provisioning?  This is feasible for those distributions
that just use the Azure Linux agent, so preferably there is a way to do
this on Ubuntu as well.

This looks like the correct problem statement - the SCSI layer schedules
these tasks and so subtle timings could cause disk names to change.
This is also we recommend UUIDs or labels or similar to be used in
fstab.

The logic for DeviceForIdePort() is based on the fact that our vmbus
driver at least knows the order of the disks (IDE or SCSI) that are
attached on the Host side.  And on Azure, the first IDE disk is always
the OS disk, and the second is always the ephemeral disk.  That is known
and must be the case for all VMs.

So in the agent, DeviceForIdePort()[0] always returns the OS disk, and
DeviceForIdePort()[1] always returns the ephemeral disk.  Ideally cloud-
init should use a similar method to auto-detect the ephemeral disk and
update fstab if needed.

[Bug 1382257] Re: [SRU] Add readme to Azure's ephemeral (resource) disk

We tested this package using the [TEST] steps above and can confirm that
the readme is created as expected.  As an additional step I also resized
the VM to ensure that the temporary disk would need to be reformatted,
and saw in this case that the readme was also created as expected.  Well
done!
