This bug occurs to me as well on a clean install of Ubuntu 15.10 (64 bits).
I have it installed on a Dell xps lalptop (I can give more details if needed).
Anyway... I've notice a very important detail. This behaviour only
happens to me when I close the laptop and it is suspended automatically.
Correction to the initial scenario: it turns out that the service
restart (quick or not) is irrelevant. It was not obvious because the
problem happens sporadically, so after adding a delay the test was
passing by pure chance.

An emphatic yes, I can replicate your described crash on my D400 when playing
the embedded video in FF that you linked to. I was able to see the ad, but as
soon as the video loaded up, my screen dropped to the black screen with the

problems with extended use, except when I attempted to hook up my

yea, in the past. it didn't help me, but because of headphones bug i had
always done many unpleasant changes. :(
anyway the alsa stable version with patch works good. though i'll try
the alsaplayer-jack firstly as well next time.
