I didn't know how to log into "Ubuntu" instead of "Ubuntu on Wayland"
since I only got a blinking cursor and no logon prompt. I edited
/etc/gdm3/custom.conf and rebooted.

Successfully got a GUI logon prompt and was able to log in to the
desktop.

I went back to the 4.18 kernel but am not running VirtualBox (running
bare metal).

I have symptoms of the failure: the same Windows instance under KVM has
disconnected from the network. I tried logging onto Windows using the
VMM console window and the wait cursor is spinning. The dmesg log file
doesn't contain any traces of the put_page issue or any other issue.
Just the usual gufw (firewall) log messages, which I am now turning off.
It's like a silent failure.

What state can I capture?

After dropping back to 3.8.0-27, reinstalling nvidia-310 and repairing
Unity, the error happened again. One of the Windows VMs loses its
network connection. When I attempt to shut the VM down (from within
Windows Server 2012), it freezes during shutdown. I am then unable to
stop the VM, not even with Force Stop.

I have since dropped back to 3.8.0-23. No errors so far after 23 hours
uptime, but it might not have been long enough.

I installed the 3.11 kernel dated 201308070514.

I had problems with:
- NVIDIA driver: was using nvidia-310, but "no suitable driver" for the newer 
kernel
- Unity: wouldn't come up
- KVM: didn't come up (machine is mainly a host for VMs)

I dropped back to 3.8.0-27 and am trying to repair Unity so I have a
desktop manager again.

Since update to 3.8.0-27.40 kernel I have been seeing these errors on an
almost daily basis. I run two Windows Server 2012 instances under KVM.
One or both of the Windows instances will disconnect from the network
when this error happens and will not reconnect until I reboot Windows.
Linux seems to recover silently on its own.

Running on an HP Z600 Workstation with ECC RAM (memory issue is very
unlikely). Scanned the boot/swap HDD for errors with Seagate SeaTools,
no issues found.
