Thanks Seth,
I can confirm after having remove the temporary hack Debian package 
firmware-bnx2, upgrading the linux-firmware package to 1.172 has fixed the 
issue. The bnx2 firmware files are there and the server comes back up after 
reboot with network interfaces working as expected.

Quick, very temporary, and very dirty hack, copy the Debian firmware-bnx2 
package to the machine and dpkg -i it:
https://packages.debian.org/buster/all/firmware-bnx2/download

netplan apply then brings the network up immediately, and only the firmware for 
bnx2 was copied.
Obviously the Ubuntu linux-firmware package ought to have it in before Bionic's 
release, but it's a quick workaround.

Just installed today's (1st March 2018) daily server CD image on a Dell
server with Broadcom BCM5716 chips and have had the same issue with the
bnx2 firmware missing, it failed on both the network configuration and
after install.

Installing that version (rsync_3.1.1-3 from the Vivid repo in this case - no

The latest package available on testing (4.9-20140213-0ubuntu1) is missing the 
/lib/x86_64-linux-gnu/libgcc_s.so.1 file.
This causes apt-get to break immediately after apt-get upgrading.
The package is unusually small:
-rw-r--r-- 1 root root 1286 Feb 14 09:38 
/var/cache/apt/archives/libgcc1_1%3a4.9-20140213-0ubuntu1_amd64.deb
at only just over 1KB
The version from packages.ubuntu.com for testing shows as 4.8.2 still, 
downloading this and dpkg-i'ing the package fixes the issue (until you apt-get 
update/upgrade again).
The old version of the file (which is 30+ times larger):

This is because the conf.d directory doesn't exist, the simple temporary 
workaround for this is to create the /etc/php5/conf.d directory, install then

Likewise, with a fresh install of Saucy, up to date as of 9th Oct 2013, the 
same thing happens.
/var/log/syslog shows JS errors saying that dbus couldn't be found in the 
search path.

I can confirm the same issue with up to date Ubuntu 12.10.
Connecting to solr gives 404s in jetty's logs.

I can confirm that after adding proposed to a 11.10 install, then apt-get 
update and do-release-upgrade'ing it to 12.04 the install now works and so far 
so good!
Hopefully it'll get into main and the netinstall soon!

Can I ask that the Precise fix gets put into 12.04.1 LTS as at the moment the 
12.04 LTS installer will not install on XenServer, this would make a big 
difference rather than having to try and install 11.10 and then upgrade (if the 
correct package will be used in the upgrade).
This is quite an urgent issue for Xen/XenServer/XCP users meaning that the LTS 
edition can't be used on a very large range of servers and virtual machines 
either by installing it directly or by upgrading.

Unfortunately this is still affecting me, and I can't get Ubuntu 12.04 LTS 
server installed on fully patched XenServer 6.0.2
I tried to get around it by installing Ubuntu 10.04.4 LTS and upgrading to 
10.10-11.04-11.10 (all fine) and then the upgrade from 11.10-12.04 failed 
with the Python upgrade crashing aborting the install with more than half of 
the packages not upgraded, and keeps crashing each time I try to apt-get 
dist-upgrade:

This may be an issue with AMD cpus, I've seen a similar problem with eglibc: 
bug 956051
I can confirm that using the Ubuntu 12.04 LTS 64bit minimal CD to do a net 
install, it gets past the initial cdrom problem, but then comes up with the 
dpkg error:

/var/cache/apt/archives/debconf_1.5.42ubuntu1_all.deb

In the log console while the installer is running (ctrl-alt-f4) I see
the following (which leads me to believe it's the above bug):

Running Kubuntu Lucid 10.04, NVidia GTX 260 graphics card, dual monitors.
I've been having this issue on and off for months, and usually rebooting lets 
me log in, but it's stuck on the splash screen and rebooting/powering off has 
not fixed it (6 times in a row).
Xorg.0.log shows:

Just to add to the above, I'm running:
kdm: 4:4.4.2-0ubuntu14
xserver-xorg: 1:7.5+5ubuntu1 (but Xorg.0.log shows: X.Org X Server 1.7.6)
uname -a shows:
Linux Beast 2.6.32-24-generic #38-Ubuntu SMP Mon Jul 5 09:20:59 UTC 2010 x86_64 
GNU/Linux

The sleep 1 workaround to /etc/init/kdm.conf didn't work the first time, 
without rebooting, making sure all X/kdm had stopped, start kdm didn't work 
and still had the same error (running via ssh).
However after a reboot it seems to be running again. I have done 5 
reboots/shutdowns in a row and it's worked each time now, looks like the 
workaround worked here =)

php5-ffmpeg Version: 0.6.0-1build3

Expected to apt-get install php5-ffmpeg.

What happened:
It tried to install apache2 when php5-cgi and nginx are installed.
When installing php5-ffmpeg the module requires the package php5, where as the 
karmic version of the package depended on php5 OR phpapi-20060613. This allowed 
installation with non apache cgi based setups such as nginx.
A simple change in the dependencies should fix this.

I've also upgraded 3 desktops with the update manager to lucid beta with no 
problems.
I have tried downgrading to the mountall karmic 1.0 package with: sudo apt-get 
install mountall=1.0 and it downgraded fine, however I still get the same error 
from do-release-upgrade -d

I've managed to around this by doing the following:

sudo sed -i 's/karmic/lucid/g' /etc/apt/sources.list
sudo apt-get update
wget 
http://mirrors.kernel.org/ubuntu/pool/main/m/mountall/mountall_2.11_amd64.deb
sudo dpkg --force-all -i /var/cache/apt/archives/mountall_2.11_amd64.deb
sudo apt-get -f install
sudo apt-get dist-upgrade

Which manually changes to lucid, gets the 64bit version of the latest
lucid mountall (change to the i386 if not running 64 bit), forces
mountall to install without the dependancies, then fixes the
dependancies and does a dist-upgrade to upgrade everything else.

On a bare-metal karmic install this worked fine, and the server has
returned after remote reboot.

The above workaround was on a linode virtual server and worked first time.
There's a slight amendment to the above instructions, the dpkg line should read:
sudo dpkg --force-all -i mountall_2.11_amd64.deb

I used the local copy that I got dist-upgrade to download while doing
testing

This bug also seems to affect 9.10 Karmic.
I can start maradns by hand without any issue, but when I reboot it doesn't 
come back up.
I've temporarily hacked this by adding this to /etc/rc.local:
service maradns restart

I can confirm this bug still exists in mysql-server-5.1 (5.1.30-2ubuntu3), from 
an upgrade of Intrepid to Jaunty, then installing 5.1.
This breaks the upgrade, however, commenting out skip-bdb fix works and apt 
continues the upgrade

Ubuntu version: 9.04 Jaunty Alpha 3 (upgraded from intrepid)
Update-notifier package version: 0.75.6

After upgrading to 9.04 Jaunty Alpha from Intrepid, and having already 
rebooted, then having done more updates only a few minutes later, the 
update-notifier shows that the machine needs to be rebooted.
Clicking the blue circular arrows brings up the dialog box, and clicking 
restart now does nothing. The dialog goes away but the machine does not 
restart, even if left for several minutes.
However, going to System - Shutdown - Restart, works fine and reboots 
straight away.
I've had this issue now several times as I've had to do several updates that 
require a reboot.

If I can do anything else to help, let me know. Thanks.

Agreed, this has affected a large amount of people, just look at the amount of 
duplicate bug reports!
I've had this issue since August and it's still a problem.
As a hack around I've had to disable auto-eth0 and setup another interface with 
a static ip, but I have to log in to get it applied, so no networking until the 
user has logged in.
I also agree, that it should have been dealt with before 8.10 was released, 
this is a major issue, could someone (maybe Dupond Jean-Louis?) increase this 
to high importance?

Ubuntu 8.10 Intrepid.
Grub: 0.97-29ubuntu45
For some time now, grub says that it will boot in 10 seconds on the current 
option, however, the countdown doesn't move (even without touching the 
keyboard).
It stays on 10 seconds, does not boot after 10 seconds, however, everything 
works as normal, up/down selects different kernels to boot and hitting enter 
boots correctly.

The following relevant lines are in the menu.lst:
default 0
timeout 10

While this isn't a huge problem for me on this local machine, when I
reboot my remote servers this would be a huge problem.

I can confirm the same problem, having just upgraded from hardy with static ip 
in /etc/network/interfaces, eth0 connects via dhcp and ignores the static 
settings.
Changing the setting through the network manager to a manual address appears to 
work for a short while, but then reverts to dhcp, even before rebooting.

I've had exactly the same problem, however just found the solution:
In the Gusty Tribe 5 live installer, open a terminal:

$ sudo su
# rmmod forcedeth
# modprobe forcedeth msi=0 msix=0
# /etc/init.d/networking restart

Your network interfaces should come back up and actually work now :-)
