The problem was not related to PS output, it was a mapping bug from
Unicode character codes to groff entities.
This is fixed now in the CVS. Thanks for the report.

disassembling the 20090104 version I see that some glyphs have different
widths, for example `.notdef' and `.null'. FontForge is correct to not
set the `isFixedPitch' flag. The same is true for the 20100919 version,
where the `.null' glyph has a different width.
Regarding FontForge:
Oops, correction: Empty glyphs don't count in the calculation of the
`isFixedPitch' flag. In the 20100919 version, glyph `acutecomb' and
other, non-empty glyphs have a different advance width.
