The gdk-pixbuf loaders.cache problem is already covered in bug #1282294
In any case it is just a warning that should not block the upgrade.

The VarLogDistupgradeApttermlog.txt contains the following:

file or directory
This likely means that your installation is broken.
Try running the command

I'm not sure which package to file against. I've used Linux for years.
How do I identify a package when it's run from some GUI package? I was
running Software Updater. It spawned something to upgrade from 14.10 to
15.04, but it doesn't identify itself w.r.t. package... The window
