on a fresh install of ubuntu 18.04 in virtualbox i can experiment this
problem since this week updates
it's a bit different from this bug report as I have this bug when adding
terminal or totem or cheese with right click, not only by drag and drop

the icon from the activities window to the doc. If you favorite the app
using right-click menu, then the icon is only added once.
