This bug also occurs for me in Hardy, using evince or kpdf. However, xpdf does not expose the problem. The same version of evince (2.22.2) in debian lenny does not have the problem, using a newer version of poppler (0.8.4 instead of 0.6.4). Can confirm this bug on Hardy. Just printed my thesis with evince and all reference are with boxes. Contents look especialy ugly, practically unreadable. The only workaround I found is to rebuild the PDF file in a way that avoids the unprintable boxes, or use Adobe Reader to print the PDF. This is happening to me too - when printing postage labels downloaded from the UK's Royal Mail. They have non-printing bits over the top - these are wrongly printed by Evince - making the labels useless, but not in Adobe Acrobat Reader for linux, v8.1.2 which behaves as expected and displays the text but doesn't print it. Here is an example of a PDF file showing the problem, with the LaTeX file that produces it.
