I'm not sure if this is a driver upgrade bug of some sort in nvidia-*,
and if so whether anyone is likely to encounter it again...
So I'll leave it Incomplete pending more reports of the same issue. If
there are none in 60 days then it will close automatically.

Confirmed that the nvidia driver wasn't selected.
Selecting/building the driver for the kernal resolved the multi-monitor issue.

You don't seem to have a kernel driver loaded for the Nvidia GPU, so any
ports attached to that GPU (like HDMI usually is) won't be usable.
This can happen if the Nvidia driver was installed and compiled for the
old kernel version but somehow failed to be rebuilt for the new kernel
version.
