Thanks for your bugreport.
This is not a apt bug, one of the internal server that hosts
archive.ubuntu.com was out of date and that is why the 404 errors
happend. It should be fully back in service now.

http://91.189.88.31/ That works, but its looking for 91.189.88.31 80
which isn't correct. There is no need for port 80, besides, there's no
':' in between the IP port
And yes, I've tried it in synaptic, ssh and ssh from
bigern75 tried it from multiple computers running hardy after a 'sudo
apt-get update' ?
Looking at the repository that it is trying to access there are no files
by those names on the server. There are similar ones however, I'm not
familiar enough with apt-get enough to troubleshoot, so I'll pass
correct
plus u can hit any of those links in bug description and they dont work.
And that matches the output of my upgrade 'error'
