This bug was never understood, and is no longer repeatable with
identical hardware. It was probably fixed, but there have been too many
changes to the kernel/device/initialisation routines in the meantime to
identify the resolution.

I disagree with the rejection. After the initial investigation, there
has been no support for this issue. I'd be happy to install the
hardware and demonstrate the failure (takes about 2 hours) if anyone
would be interested in troubleshooting it.

The closest package I can find for this is initramfs-tools. Hopefully
the maintainers of that package or someone more knowledgeable than I am
will be able to continue discussion on this bug.

Actually, it always worked fine with initrd-tools. The regression
occurred with the upgrade of initramfs-tools from 0.24 to 0.25. Looking
at the changelog again, it appears that a possible temporary workaround
was included in 0.40ubuntu5: I'll test again this evening and attach
previously
