Reopening, this is the desired behaviour. Previously save always
prompted for the location to save to. Now it behaves like other
applications where save overwrites the current document, and save as
saves to a new document. The solution is probably to indicate in the
title bar if it is unsaved

From running simple-scan in debug mode -d it shows that it does
actually save, but it automatically overwrites the previously saved
file.
