no idea where it comes from but that message is from a GTK application (KDE HIG define the approving button left and the rejecting button right, GTK is the other way around). Also the polkit binary you mentioned is nothing but an authorization helper, but certainly not the application creating the

Screenshot showing the dialog. The weird effect on the dialog's backgorund is because the dialog is semi transaparent (which according to my kwin settings probably means its a dialog and not a normal window) and it is disaplayed over a part of the background that has a sharp brightness change.
