I spent some time digging into the libunwind test failures; from the
large amount of discussion about failing tests on the mailing list it
appears that there are an expected set of failures which vary depending
on architecture.
On x86_64 (5 failures); I was able to cherry pick two fixes
Due to note 3, I assume, that note 4 is irrelevant and just a byproduct
of marking the issue duplicate/invalid.
According to note 3 and if applicable, please add reference to the
kernel bug report after creation, so that I can start tracking that one
instead.

I'm not convinced that the deadlock issue in google-perftools is not
caused by the combination of libunwind/glibc that is present in 12.10.
The INSTALL, bug tracker and ML make reference to this being a pain
point on x86_64 - but I see the same issues on other archs.

Hmm, we've got some counter indicators here.
lvs claims that the volumes are active. But the probe itself
is showing problems reading the volumes.
XFS is telling us that it cannot write it's journal to disk.

scratch the ext3 test, I just re-read the description.
I've replicated this behaviour using both xfs and ext4 filesystems, on multiple
different luns presented to the server.

API 'v1.1/{tenant_id}/os-hosts' does not return a list of hosts.
Instead it returns an empty set.
This is a known bug and was fixed for Folsom. However it has not been fixed for
Essex. Last good version was 2012.1-0ubuntu2.3. Link to nova bug:
