Public bug reported:
During installation, i got an error saying 'input/output' error and then
it said that the installer has crashed and report will be sent to
ubuntu. and then i was directed to this page to submit a bug report
This happened after i selected a partition to format as ext for and to
mount it as / and clicked on 'continue' button
